HOUSING ASSISTANCE FORMS & APPLICATIONS
Tenant-Based Rental Assistance Application
Submit an application to apply for rental assistance.
*Note: This form is only available during October 1 through October 31 each year. A Release of Information Authorization Form is also required to be completed by all members of the household.
Apply for available low-income housing units, awarded through a point-based waiting list.

HOMEOWNERSHIP ASSISTANCE FORMS & APPLICATIONS
Submit an application for down payment assistance for Federally Recognized enrolled Native Americans.
*Note: A signed Down Payment Assistance Program Policy is also required by the Borrower (and Co-Borrower). A Release of Information Authorization Form is also required to be completed by all members of the household.

WHAT IS THE PROCESS FOR APPLYING FOR HOUSING ASSISTANCE?
We can mail you a physical application, or you can apply online through our website. Submitting your application online is generally faster and more convenient.
HOW DO I RECERTIFY MY TENANT OR APPLICANT INFORMATION?
NPHA will either mail out a recertification form or complete the recertification over the phone.
WHEN CAN I APPLY FOR THE TENANT-BASED RENTAL ASSISTANCE PROGRAM?
The application for Tenant-Based Rental Assistance Program is open October 1 and the deadline is October 31 each year.
HOW CAN I SUBMIT FEEDBACK TO NPHA?
